In opting for any of the payment plans, I fully understand that I am agreeing to allow CAI Medical Transcription LLC to automatically process subsequent monthly payments according to the payment plan I select on the credit/debit card I provide to the school. By selecting a payment plan and providing CAI with a credit/debit card to automatically charge monthly payments, I understand and agree to the following: In opting for any of the payment plans, I am aware that should any subsequent monthly payment be declined for any reason, a $20.00 decline fee will be charged for each occurrence and CAI will notify me via phone and/or email. I have 24 hours to contact the school and provide optional credit/debit card information in order for my monthly payment to be processed. Should I not contact CAI within the 24-hour period to provide optional payment information or not receive notification because of extenuating circumstances, I am aware that I will then be charged a $35.00 late fee. In the event my account remains delinquent at the time my next monthly payment becomes due, that my account will be turned over to a collection agency. Once my account has been referred to a collection agency, I am aware that I will then be responsible to pay any outstanding balance on my account immediately including any late/decline fees and/or collection processing fees assessed by CAI. Once that has occurred, CAI will have no further obligation to you as a student and all further correspondence will be done via the collection agency. I will provide CAI with up to date contact information providing them the ability to contact me should a decline occur. If contact information is invalid or I do not receive the notification via phone and/or email for whatever reason, CAI will not be responsible and the aforementioned collection steps will ensue. Should I opt to provide CAI with a checking account debit card, in the event money is not available at the time the monthly payment is processed, that I do possess a valid credit card as an alternative means to process payment. I also understand that CAI will not be responsible should your checking account become withdrawn due to the automatic monthly payment being made under any circumstance. CAI will provide me with a monthly date on my invoice, which will be included in my package that the payment will be automatically withdrawn. In the event my date falls on a weekend or holiday, the payment will be withdrawn one business day prior to that date. I fully understand that should I not abide by these payment plan guidelines for whatever reason, CAI has the right to void the payment plan agreement and require payment of the remaining balance on my account in full.
