In opting for any of the above payment plans, I fully understand that I am agreeing to allow CAI Medical Transcription LLC to automatically process subsequent monthly payments according to the payment plan I select above on the credit/debit card I provide to the school. I also fully understand that in opting for any of the above payment plans, I am aware that should any subsequent monthly payment be declined for any reason, a $20.00 decline fee will be charged for each occurrence and CAI will notify me via phone and/or email. I further understand that I have 24 hours to contact the school and provide optional credit/debit card information in order for my monthly payment to be processed. Should I not contact CAI within the 24-hour period to provide optional payment information or not receive notification because of extenuating circumstances, I am aware that I will then be charged a $35.00 late fee. I also understand that in the event my account remains delinquent at the time my next monthly payment becomes due, that my account will be turned over to a collection agency. Once my account has been referred to a collection agency, I am aware that I will then be responsible to pay any outstanding balance on my account immediately including any late/decline fees and/or collection processing fees assessed by CAI. Once that has occurred, and I contact CAI within 60 days from the date of the original decline wishing to continue my enrollment, I fully understand that the total remaining balance on my account including any decline/late/collection fees plus a re-enrollment fee (to be determined according to tuition guidelines at that specific time) will be required at that time in order to continue the course. I also understand that after 60 day's time, no further options will be available and collection will be continued through the collection agency, and CAI will have no further obligation to me as a student. I also fully understand that I will provide CAI with up to date contact information providing them the ability to contact me should a decline occur. If contact information is invalid or I do not receive the notification via phone and/or email for whatever reason, CAI will not be responsible and the aforementioned collection steps will ensue. Furthermore, I also fully understand that if I opt to provide CAI with a checking account debit card, in the event money is not available at the time the monthly payment is processed, that I do possess a valid credit card as an alternative means to process payment. I also fully understand that should I not abide by these payment plan guidelines for whatever reason, CAI has the right to void the payment plan agreement and require payment of the remaining balance on my account.
